Human error is a major source of risk and loss in facilities. In many cases of loss due to human error, errors can be related to several key factors , including; internal factors such as fatigue, or external factors such as the man machine interface design, or designs of jobs and procedures, or issues with the systems that support the undertakings. In fact, there could be contributions from all the elements.

This course helps to understand how human error can lead to an incident or an accident, and the importance of human factor assessment during the design of systems, processes and even equipment with safety critical applications.
